As you can see in the above image (I've marked them out) there are several errors with custom shortcuts in the Recent list.

Two of the entries have the wrong icons listed against them and one has no name at all. (Yes, they are all custom shortcuts)

And yet, the StarCraft icon remains correct in the Recent list.

Quite unusual bug, just thought you might like to know.

Quickedit: Changed from tree to large icons and the bug still occured.

Quickedit 2: Tried to open the properties of the Counter-Strike shortcut from the Recent menu...got WinMerge's properties window instead. WTF?

Quickedit 3: Tried to open Quake 3 and Starcraft and Age of Empires' shortcut properties as well, they work fine.

Maybe an idea.. Did you edit manually, or restore a backup of your xml files stored in the folder "LiberKeyTools\LiberKeyMenu\data\Menu" ?
Can you send me these xml files to my email ( vagabond ) for analysing it?
